At Age Well Therapy we have a special interest in dementia. Occupational Therapists have a lot to offer people living with dementia and their families, through comprehensive occupation-focused assessments, therapy interventions and carer education and support.
The journey of dementia is different for each person diagnosed and our role is to provide clients and their families with the 'tools' needed to preserve their memory and function for as long as possible. Working together, the person with dementia can be supported to participate in meaningful activities to a level which is comfortable and achievable, which in turn builds self-esteem and can reduce dependency on others. Age Well Therapy can support you at any point in your dementia journey and can tailor a program with practical solutions to reduce your dementia symptoms and help you maintain meaningful relationships with those you love.
Age Well Therapy are proud to be able to deliver COPE (Care of People with dementia in their Environments), an evidence based Occupational Therapy program for people living with dementia and their care partners. COPE is a practical program designed to enhance the wellbeing and quality of life of both the individual with dementia and their carer through education and creative problem solving.
Over 10 sessions, COPE provides carers with valuable skills to prevent and manage challenging dementia symptoms, which enables the person with dementia to remain living well at home for longer. Further information on the COPE program can be found at https://copeprogram.com.au/
Functional cognition is the ability to use thinking and performance skills to accomplish everyday activities like making a meal and paying a bill. At Age Well Therapy we conduct comprehensive functional cognition assessments that identify your current impairments and preserved abilities.
Understanding your current cognitive capacity shouldn’t cause you to stop activities or give up parts of life that are important to you, rather it will enable you and your family to appropriately modify the task and your environment, so you are set up for maximum independence and success.
Personal Activities of Daily Living (PADL’s) are the basic tasks we complete every day. They include showering, dressing, toileting, eating and walking. At Age Well Therapy we prioritise your strengths and preserved abilities instead of your impairments. We can recommend different ways of completing activities that maintain your safety and independence.
We all have an instinctive need to ‘do’ and contribute, and that doesn’t change as we age. When an individual has difficulty or is not able to complete a particular task due to injury or disease, it is common for others to take over and complete tasks for them. Even if an individual is assisted with tasks like showering or dressing, we can identify parts of the activity they are able to complete safely on their own to maintain their self-worth and identity.
Instrumental Activities of Daily Living (IADL’s) are the complex everyday functional activities that are necessary to live independently, such as managing your medications, purchasing groceries, or paying a bill. At Age Well Therapy we can assess the way you complete these activities and recommend strategies to improve your safety and independence.
Preserved IADL is one of the defining features distinguishing mild cognitive impairment (MCI) from dementia. A performance-based IADL assessment can ensure you receive the right support early to preserve your abilities for as long as possible.
The home environment can make completing necessary and important tasks very difficult. Our Occupational Therapists assess areas of your home that are challenging and prioritise low cost and effective solutions to make your life easier.
These may be as simple as a grab rail beside your toilet or more complex like a ramp to enable level entry into your home.
Assistive Technology (AT) is an umbrella term for any piece of equipment, software program or system that provides practical solutions for everyday activities. At Age Well Therapy we can help you find the right piece of equipment to suit your individual needs.
We know that sometimes managing tasks around the home can be difficult. We provide assessment, trial and training in the use of assistive equipment. New technologies (like home automation) can also play an important role in simplifying your everyday tasks and help you live safely and independently within your own home longer.
Have you recently had a fall, or do you have concerns about your safety within your home? We offer comprehensive home safety assessments to reduce your future falls risk and ensure your home environment is as safe as possible.
All of us want to stay active and independent and live in our own homes for as long as possible. Falls do not have to be an inevitable part of growing older – you can reduce your falls risk by participating in a home safety assessment. During a home safety assessment, we will help you understand your personal risk factors and offer solutions to manage these, teach you how to keep yourself safe by identifying and removing hazards around your home and leave you with a personalised action plan to maximise your safety and confidence into the future.
